Care Supreme vs Optima Secure: Which Health Cover Actually Grows With You? Two plans, one promise, two very different routes
| AI OVERVIEW |
| Care Supreme (Care Health Insurance) and Optima Secure (HDFC ERGO) both offer no room rent limit, no standard co-pay, 60 days pre- and 180 days post-hospitalisation cover, and the IRDAI-standard waiting periods (30 days initial, 24 months for listed illnesses, 36 months for pre-existing diseases). They grow differently. Optima Secure front-loads cover: Secure Benefit adds 100% of the base sum insured from day one, Plus Benefit adds 50% a year up to 100%, Automatic Restore refills 100% of the base once a year, and Protect Benefit pays 68 listed non-medical items, all built in. Care Supreme starts leaner: a 50% yearly cumulative bonus up to 100% and Unlimited Automatic Recharge, with optional add-ons such as Cumulative Bonus Super (100% a year up to 500%), Claim Shield for non-payable items, and Instant Cover for diabetes, hypertension, hyperlipidaemia, and asthma. In both, bonuses aren’t reduced by claims, and restored cover can’t be used to stretch a single claim. Compared on the policy wordings: my: Optima Secure (UIN HDFHLIP25041V062425) and Care Supreme (UIN CHIHLIP27061V032627). |
Both plans make the same pitch: buy once, and your cover keeps growing without you lifting a finger at renewal.
It’s a good pitch. Medical costs in India have been rising at roughly 12% to 14% a year (a widely cited industry estimate, not an official figure), so a cover that stays flat quietly shrinks in real terms.
But “grows with you” means different things in these two policies. One hands you most of its growth on day one and caps it early. The other starts smaller and lets you buy much taller growth as an add-on. And the fine print on single claims, restoration, and co-pay options decides what actually gets paid. Here’s the comparison, clause by clause, straight from the policy wordings.

(All features as stated in the two policy wordings unless noted. The policy schedule and wording issued to you are the contract.)
Room rent: no cap on either side, but watch the options
- Optima Secure pays room rent and ICU at actuals.
- Care Supreme states the eligible room category and ICU charges as “No limit” in the base plan.
The catch is on Care’s optional menu: Room Rent Modification limits you to a Single Private AC room or a Twin Sharing room for a lower premium. If you’re then admitted to a costlier room, the wording applies a proportionate deduction to associated medical expenses (room, nursing, OT and doctor fees), though pharmacy, consumables, implants and diagnostics are excluded from that deduction, as IRDAI’s standard definitions require. ICU stays uncapped even with this option. See room rent limits explained.
Co-pay: none by default, but Care has discount-for-co-pay options
- Neither base plan carries a co-pay. Optima Secure goes further: its wording states no co-payment applies if a Tier 2 resident takes treatment in a Tier 1 city (Tier 1 being Delhi NCR, Mumbai region, Surat, Ahmedabad and Vadodara).
Care Supreme offers three optional ways to trade a co-pay for a lower premium. Know what you’ve ticked:
| Care Supreme option | What it does |
| Smart Select | Premium discount, but a 20% co-pay on every claim at hospitals outside the insurer’s listed network (Annexure III) |
| True Connect | Premium discount, but a 10% co-pay on non-emergency claims if you skip the first consultation with the insurer’s empanelled doctors |
| Co-payment | You choose a co-pay percentage on every claim, for a discount |
See co-payment explained.
Waiting periods and chronic conditions
Both wordings use the IRDAI-standard structure: 30 days initial (accidents covered from day one; the 30-day rule doesn’t reapply after 12 months of continuous cover), 24 months for a listed set of illnesses and surgeries, and 36 months for pre-existing diseases, with credit for prior continuous cover when you port. Both lists include cataract, hernia, joint replacement, kidney and gall bladder stones, piles and fistula, and benign ENT surgeries; Care’s list also includes Parkinson’s, Alzheimer’s and dementia.
The shortcuts differ:
- Care Supreme, Instant Cover: waives the PED wait for diabetes, hypertension, hyperlipidaemia and asthma, with cover from day 31. It’s available only on your first Care policy, and you can choose it or the PED Wait Period Modification, not both. Care also offers a Named Ailment Wait Period Modification.
- Optima Secure, ABCD Chronic Care add-on: covers hospitalisation for asthma, blood pressure, cholesterol and diabetes after the 30-day wait. Optima Secure’s own PED wait modification (to 1 or 2 years) can only be set up at channel level, so an individual customer can’t opt for it directly.
See waiting periods explained.
How each plan grows: bonus, day-one boost and restoration
Care Supreme
- Cumulative Bonus (built in): 50% of the sum insured for each completed, continuous year, up to 100%. Claims have no impact. It’s forfeited if you don’t renew by the end of the grace period.
- Cumulative Bonus Super (optional): a further 100% of the sum insured per year, over and above the cumulative bonus, up to 500%. Claims have no impact; it’s forfeited if you drop the option at renewal.
- Plus Benefit (optional): an extra amount (as per your schedule) usable after the base sum insured is exhausted.
- Unlimited Automatic Recharge (built-in): reinstates up to the base sum insured unlimited times a year, for the same or different illnesses and any insured member, but only after the base, bonuses and Plus Benefit are fully used.
Optima Secure
- Secure Benefit (built in): 100% of the base sum insured, usable after the base is exhausted, once each year (not carried forward).
- Plus Benefit (built in): 50% of the base sum insured added at each renewal, up to 100%, irrespective of claims.
- Automatic Restore (built in): instantly adds 100% of the base sum insured when a claim partly or fully uses it up, for later claims that year, including the same illness and any member. It triggers once a year unless your schedule says “Unlimited Times” (the Unlimited Restore add-on).
The single-claim rule both plans share
Restored or recharged cover pays later claims. It can’t be used to stretch one big claim:
- Care Supreme: a single claim can’t exceed sum insured + cumulative bonus + Cumulative Bonus Super + Plus Benefit + Unlimited Care (if opted).
- Optima Secure: a single claim can’t exceed base sum insured + remaining cumulative bonus/Plus Benefit + remaining Secure Benefit.
Case study: how ₹10 lakh grows, and what one big claim gets
Illustrative, assuming a ₹10 lakh base, continuous renewals (bonuses in both plans accrue whether or not you claim), and no deductible.
| Policy year | Care Supreme (base) | Care Supreme + Cumulative Bonus Super | Optima Secure |
| Year 1 | ₹10L | ₹10L | ₹20L (base + Secure) |
| Year 2 | ₹15L | ₹25L | ₹25L |
| Year 3 | ₹20L (bonus cap) | ₹40L | ₹30L (Plus cap) |
| Year 6 | ₹20L | ₹70L (Super cap) | ₹30L |
| Plus, for later claims in the year | Unlimited recharge of ₹10L | Unlimited recharge of ₹10L | One restore of ₹10L (unlimited with add-on) |
One ₹35 lakh cancer claim in Year 3:
| Plan | Single-claim maximum | Insurer pays | Family pays |
| Care Supreme (base) | ₹20L | ₹20L | ₹15L |
| Care Supreme + Cumulative Bonus Super | ₹40L | ₹35L | ₹0 |
| Optima Secure | ₹30L | ₹30L | ₹5L |
(Subject to all other policy terms. A super top-up is another way to cover a big single claim.)
The takeaway: Optima Secure gives you more cover sooner, but its growth stops at 3x (4x counting one restore). Care Supreme’s base plan grows less, but with Cumulative Bonus Super it can reach 7x by year six, at an extra premium.
Consumables and non-medical items
- Optima Secure builds in Protect Benefit, which pays for the 68 non-medical items listed in Annexure B (such as gloves, masks, nebuliser kits and attendant charges) when the main claim is admissible.
- Care Supreme offers Claim Shield as an option: items that are otherwise non-payable under List I of Annexure I become payable, up to the sum insured.
Deductibles: both offer them, with trade-offs
Both plans let you opt for an aggregate deductible (applied once a year across claims) for a lower premium.
- Optima Secure: options from ₹10,000 to ₹25 lakh (₹5L and ₹10L need a sum insured of at least ₹25L; ₹20L and ₹25L need at least ₹50L). Choose ₹10 lakh or more, and you lose Secure Benefit, Plus Benefit, Automatic Restore, daily cash, and the health check-up. You can reduce or waive the deductible once, after 5 continuous years, if the eldest member was under 50 at purchase and is under 61 at the time.
- Care Supreme: deductible per your schedule, applied on an aggregate basis, with a premium discount.
Extras that separate them
| Add-Ons | Care Supreme | Optima Secure |
| Doctor access | Unlimited GP e-consultations (built-in) | E-opinion for 51 listed critical illnesses (built-in) |
| Health check-up | Optional annual check-up | Built-in preventive check-up from year 2 (e.g. ₹2,000 individual / ₹5,000 floater at ₹10L) |
| Home treatment | Domiciliary hospitalisation | Home health care (cashless, pre-authorised) and domiciliary hospitalisation |
| Air ambulance | Optional | Built in, up to ₹5L, without reducing the sum insured |
| Shared-room incentive | Not offered | ₹800/day up to ₹4,800 for stays over 48 hours in a shared room at a network hospital |
| Wellness | Optional: renewal discount up to 30% for 270+ days a year of 10,000 steps | Not in the wording |
| One unlimited claim | Optional Unlimited Care (once in the policy lifetime; opt at inception; keep 5 years) | Limitless add-on (specified number of claims of unlimited value, India only) |
| Newborn | Optional newborn cover from day 1 | Parenthood add-on (maternity, embryo freezing, IVF after a 2-year wait) |
| OPD-style covers | Optional Women Care and Mental Health Wellbeing | Optima Wellbeing add-on |
Rules both plans share (because IRDAI requires them)
| Rule | Both wordings say |
| Grace period | 30 days (15 for monthly); cover during the grace period only if you pay in instalments |
| Free look | 30 days from receiving the policy, for new policies only |
| Claim settlement | Within 15 days of the last necessary document; delay interest at 2% above the bank rate |
| Moratorium | After 60 continuous months, no contesting for non-disclosure except established fraud |
| Renewal | Can’t be refused for past claims; no loading based on your individual claims |
| Portability | Apply 30 to 60 days before renewal |
CoverSure Nugget: read the single-claim cap before the headline multiple“7x” and “4x” sound like cover you can spend on one hospital bill. They aren’t. In both plans, recharged or restored money only pays later claims in the year. For one catastrophic bill, what counts is base cover plus accumulated bonuses (and, in Optima Secure, the Secure Benefit). Check that number first. |
